NEW ORLEANS (AP) - A health center in New Orleans' Bywater neighborhood has received $550,000 to support both coordinated doctors' care for area residents and a neighborhood outreach program.
United Health Foundation gave the grant Wednesday to the Daughters of Charity Health Centers' St. Cecelia location.
The foundation has given nearly $7 million in grants to Daughters of Charity Health Centers since 2006.
The system uses teams of doctors, nurse care managers, behavioral health managers and case managers to provide long-term, comprehensive and coordinated care for patients, so they don't shuttle among specialists who don't know their medical history.
The Neighborhood Health Partnership provides health screenings and education. It also directs people to Daughters of Charity Health Centers for primary and preventive care.
